Smilz Referral Program – Terms & Conditions

Patient Loyalty Program
  1. Anyone can register with this Referral Program. On completion of registration you will get 100 points, which is equal to 100/- Rupees and you can use it for cash back on consultation or dental procedures of your choice. This is only available once. It is associated with your email id and your mobile number, which need to be validated when you visit our clinic. 
  2. On Registration, you will get a shareable link on your dashboard,d which you can use to share with your family and friends or anyone you know personally. Once they register through your link, you get 50 points and your referred person gets 100 points on successful completion of registration. Every user and referral should have a unique email address.  Email once registered with one user cannot be used for any other user.
  3. You can refer any number of persons, but you will get 50 points only after the successful registration of every user.
  4. You can continue earning points with such referrals and use it in future for our treatments.
  5. Every new registration, whether through referral or direct, will get 100 direct points = 100/- rupees.
  6. The points that you get for your referrals will be accumulated in your account and can be used ONLY after your referred persons visit Smilz for consultation or treatments. 
  7. Once your referred person visits Smilz for consultation or Treatment, you will get another 50 points credited to your account from our end. 
  8. Anyone can use up to 500 points as a discount for consultation ( X-Rays not included in discounts) and 20% of the value of the treatment cost at the maximum, whichever is lower. 
  9. ( For example suppose you have 1500 valid points and your total bill is 5000/- rupees including consultation, of which 500/- is the consultation fees. Then your procedure cost is 4500/- rupees. So your discount will be:

500/- + 20% of 4500/-(900/-) = 1400/-. You still have 100 points (e.i, 100/- Rupees left for future use.) If your valid points are 1000 then your discount is 500/- for consultation and 500/- from 4500/-. For any clarification you can whatsapp us at:
